What is gynecomastia?

Gynecomastia (also written gynaecomastia in UK clinical practice) is enlargement of male breast tissue, caused by glandular tissue, excess fat, or usually both. It's common: it affects a large proportion of men at some point, often appearing in puberty and sometimes persisting, or emerging later with weight change, age, certain medications or hormonal shifts. It is rarely a sign of anything serious, and it is not your fault, or your gym programme's.

The distinction that shapes treatment: glandular tissue feels firm, sits behind the nipple, and does not respond to exercise or weight loss; fatty tissue softens the whole chest and partially responds. Because most men have a mix, most surgery combines two techniques. Assessment first also screens the small number of cases with an underlying cause worth treating (medication effects, hormone issues), which is what makes a medical consultation the right first step rather than a gym plan or a cream.

Who surgery is right for

Good candidates are at a stable weight, carry tissue that has persisted despite fitness, have realistic expectations, and have physiological gynecomastia confirmed at assessment. The honest screens matter as much as the green lights. Where fat dominates and weight is still moving, significant weight loss comes first, because it may solve the problem or shrink the operation. Medications and hormones are reviewed, since treating a cause beats operating around one. And your grade is assessed honestly, because higher grades with loose skin change the operation from a discreet day case to something involving skin removal, which deserves an upfront conversation, not a surprise.

Teenage gynecomastia is usually observed rather than operated, because most of it resolves on its own. And if your chest would be better served by weight management or by treating an underlying cause, that's the advice you'll get.

Liposuction, gland excision, or both

Liposuction removes the fatty component through tiny incisions. Gland excision removes the firm disc of glandular tissue through a small incision at the edge of the areola, where the scar hides in the colour change. Most cases combine both for a flat, natural, masculine contour; higher grades with loose skin may need skin-tightening approaches, discussed openly if that's your anatomy. The goal is a chest that looks unoperated, in a t-shirt and out of one.

How Gynecomastia Surgery Works at Apex

The garment for around four to six weeks (it does real work; wear it); desk work at about a week; driving when comfortable, typically one to two weeks; gym lower body from two to three weeks, chest training from four to six; the settled result at three months, reviewed with your surgeon.

The compression garment is the unglamorous hero of this operation: it does real work shaping the result, so wear it as prescribed, typically four to six weeks. Expect the chest to look bigger before it looks smaller, because the first week belongs to swelling; that's normal and temporary. Numbness around the nipples recovers over weeks to months for most men.

Scars are cared for like any fine surgical scar, protected from sun while they mature, and typically fade into the areola's colour boundary. As for permanence: removed gland does not regrow. Significant weight gain, certain medications or hormonal changes can enlarge the tissue that remains, so stability protects the result, and for most men one operation is the end of the story.

Yes; gynecomastia is the medical name for it. The informal name trivialises something that affects confidence profoundly, which is why we use the medical one.

Because glandular tissue doesn't burn. If a firm disc sits behind the nipple, no volume of pressing will remove it; that's precisely the tissue surgery excises.

Liposuction leaves millimetre marks in hidden creases; gland excision leaves a fine scar along the lower edge of the areola that typically fades into the colour boundary. Most men's scars are effectively invisible at conversational distance within months.

The removed gland does not grow back. Significant weight gain, certain drugs or hormonal changes can enlarge remaining tissue, so stability preserves the result; for most men, one operation is the end of the story.

Rarely. One-sided, hard or otherwise unusual breast tissue in men deserves proper assessment, and that screening is part of the consultation. It's another reason to see a surgeon rather than self-diagnose.

Pubertal gynecomastia usually resolves on its own within a couple of years, so the standard advice for teens is assessment, reassurance and patience rather than early surgery. We'll advise honestly.
