Good candidates are at a stable weight, carry tissue that has persisted despite fitness, have realistic expectations, and have physiological gynecomastia confirmed at assessment. The honest screens matter as much as the green lights. Where fat dominates and weight is still moving, significant weight loss comes first, because it may solve the problem or shrink the operation. Medications and hormones are reviewed, since treating a cause beats operating around one. Anchor or lollipop, chosen by your anatomy
Two established incision patterns cover most reductions. The lollipop (vertical) pattern, a circle around the areola with a line down to the breast crease, suits moderate reductions and leaves less scar. The anchor (inverted-T) pattern adds a line along the crease itself, and is the workhorse for larger reductions, where more skin has to be reshaped.
In both, excess tissue and skin are removed, the breast is reshaped and lifted, and the nipple is repositioned to sit naturally on the new contour; liposuction occasionally assists, but excision does the real work.
