What is breast reduction?

Breast reduction (reduction mammoplasty) is surgery to remove excess breast tissue, skin and fat, reducing the size and weight of the breasts and reshaping them in the same operation. Most patients choose it to relieve physical symptoms: neck, back and shoulder pain, posture problems, bra strap grooves and skin irritation, alongside the practical toll on exercise and clothing. Surgery takes around 2.5 to 3 hours under general anaesthetic, as a day case or with one overnight stay.

The reshaping is not an afterthought: the operation lifts as it reduces, repositioning the nipple and restoring a more youthful contour, so the result is smaller, lighter and lifted in one. What you're really buying is proportion, and the end of a daily negotiation with your own chest.

Who surgery is right for

Good candidates are at a stable weight, carry tissue that has persisted despite fitness, have realistic expectations, and have physiological gynecomastia confirmed at assessment. The honest screens matter as much as the green lights. Where fat dominates and weight is still moving, significant weight loss comes first, because it may solve the problem or shrink the operation. Medications and hormones are reviewed, since treating a cause beats operating around one. And your grade is assessed honestly, because higher grades with loose skin change the operation from a discreet day case to something involving skin removal, which deserves an upfront conversation, not a surprise.

Teenage gynecomastia is usually observed rather than operated, because most of it resolves on its own. And if your chest would be better served by weight management or by treating an underlying cause, that's the advice you'll get.

HOW IT'S DONE

Anchor or lollipop, chosen by your anatomy

Two established incision patterns cover most reductions. The lollipop (vertical) pattern, a circle around the areola with a line down to the breast crease, suits moderate reductions and leaves less scar. The anchor (inverted-T) pattern adds a line along the crease itself, and is the workhorse for larger reductions, where more skin has to be reshaped.

In both, excess tissue and skin are removed, the breast is reshaped and lifted, and the nipple is repositioned to sit naturally on the new contour; liposuction occasionally assists, but excision does the real work. Which pattern you're offered is decided by your anatomy and the reduction you want, not by a one-size approach, and you'll understand the scar trade-off before you decide.

Patients arrive asking how much smaller. The better question, answered at consultation, is what proportion looks and feels right on your frame, because the operation is a reshaping that reduces, not an amputation of volume.

ONGOING CARE

Recovery, week by week

Week one brings moderate swelling and bruising, a support bra worn continuously, and, for many patients, an immediate and striking absence: the weight is gone, and backs and shoulders notice before anything else heals. Desk work is possible for most within the first week or two.

Weeks two to three, pain recedes and light daily activity builds back. Weeks four to six, scars begin fading, light exercise returns, and the shape starts settling; strenuous exercise waits until around six weeks with your surgeon's go-ahead.

The final shape settles over three to six months as swelling resolves, with scars continuing to mature and fade well beyond that. Your aftercare plan puts all of this in writing, with consultant-led support throughout.
